대리외상후 성장 연구의 동향과 과제-상담자 및 정신건강전문가를 중심으로-

Abstract

간접적 외상 노출에 대처하는 과정에서 이루어지는 상담자의 개인적․전문적 성장이 2000년대 이후 활발하게 보고되고 있으나, 이에 관련된 국내․외 문헌들에 대해 통합된 정보를 주는 연구는 찾아보기 어렵다. 따라서 본 연구의 목적은 대리외상후 성장과 관련된 문헌들 중 상담자 및 정신건강전문가를 대상으로 이루어진 연구들을 선별하여, 그 전반적인 흐름을 제시하는 것이다. 이를 위해 19편의 논문이 선택되었고 네 가지 영역, 즉 측정도구, 외상후 성장과의 비교, 관련 요인, 대리외상 및 이차적 외상스트레스와의 관계로 분석하였다. 분석 결과 첫째, 대리외상후 성장을 측정하는데 외상을 경험한 대학생들을 대상으로 개발된 외상후 성장 척도가 주로 사용되었고, 인지적 변화에 대한 부분을 보완하기 위해 관점의 변화 척도를 추가하거나 대리외상에 대한 반응을 포함시켜 측정하려는 시도가 있었다. 둘째, 대리외상후 성장과 외상후 성장은 그 특징과 인지처리과정에 있어서 공통점이 있으나 대처과정, 전문성 발달, 공감과 역할의 사회적 확장이라는 측면에서는 분명한 차이를 드러났다. 셋째, 대리외상후 성장에 관련된 요인으로 상담사례 수, 심리적 요인, 사회적 지지와 지원, 인구통계학적 변인이 다루어졌으며 복합적인 결과를 제시하고 있었다. 넷째, 대리외상후 성장과 대리외상 및 이차적 외상스트레스(VT/STS)와의 관계는 VT/STS가 증가하면서 선형으로 증가하다가 성장이 정점에 오르면 VT/STS가 계속 증가하더라도 성장 곡선은 감소하는 형태를 보이는 것으로 나타났다. 이상의 내용에 기초해 분석된 결과에 대한 논의와 더불어 후속연구를 위한 제언을 덧붙였다.

Although personal and professional growth of counselors in the process of coping with indirect trauma exposure has been actively reported since the 2000s, it is difficult to find research that provides a broad review of the available literature, domestic and foreign, on this issue. The purpose of this study is to present the overall trend in vicarious posttraumatic growth (VPTG) of counselors and mental health professionals by reviewing current domestic and foreign studies. For this task, 19 articles were selected and analyzed in respect to four areas: measurement tools, comparison with posttraumatic growth (PTG), related factors of occurrence, and relationship with vicarious trauma (VT) and secondary traumatic stress (STS). The results were as follows: First, PTGI was mainly used to measure VPTG and additional measurement tools for the cognitive change and responses to VT. Second, VPTG shared similar features and generators with PTG, but there was a clear difference in coping process, in developing the expertise and in expanding empathy and social role. Third, the degree of exposure to indirect trauma, related psychological factors, available social support and demographic variables were all related to VPTG. Fourth, the relationship between VPTG and VT/STS showed a curvilinear curve (reverse U). Finally limitations on the current literature and areas for future research are suggested.
